A pharmacist can help patients with uncontrolled hypertension by teaching them how to monitor their blood pressure at home and encouraging medication adherence. If state law permits collaborative agreements, the pharmacist may also be able to adjust the therapy to reach blood pressure goals. There are many other ways a clinical pharmacist embedded in a practice can contribute to patient care.
